Most action plans fail because the actions describe intentions and nobody owns them. This template starts from one goal with measures, then breaks it into verb-led actions that each have a single owner, a due date and a check. It is organised as a goal scorecard, an action list, a RACI matrix, a 12-week timeline, a measures page and a definitions page.
The sample follows Ivywell Partners as it cuts client onboarding from 14 days to 10 between November 2, 2026 and January 22, 2027. It shows quick wins in the first two weeks, process fixes in the middle and a final verification. Replace the goal, owners, dates and figures with your own, and keep the one-owner rule.
Click Make a copy in Google Docs, then confirm on the Google page. The copy is saved to your own Google Drive.
State your goal in one sentence with a number and a date, then enter the baseline and target for each measure on the first and measures pages.
Rewrite each action to start with a verb and give it one owner. Put at least one due date within the first two weeks and spread the rest.
Update the RACI matrix so every task has exactly one accountable role, and edit the timeline weeks to match your dates.
Delete the Make it yours guide page, then share the plan or present it at your next review.
Yes. Replace the goal with the issue, make the first actions the corrective ones, add preventive actions, and set the verification date 30 to 90 days after the last action closes.
Six to fifteen works for most goals, each with one owner and a due date between two and twelve weeks out. More than that usually means two goals are mixed together.
No. Ivywell Partners, its people, email addresses and every figure are fictional sample content. Replace all of it with your own information before you share the plan.
