A performance conversation fails when expectations are vague and support is promised but not scheduled. This action plan puts the concerns, the measurable expectations, the people who will help, the review dates and the outcome process into one document that the manager, HR and the employee can all read and sign.
The sample follows Hartwell Partners and a senior account manager over a 12-week plan from March 15 to June 7, 2027. It shows five rated concerns, five measures with baselines, six named supporters and a day-60 scorecard. Replace the concerns with facts you can evidence, then set each target and review date.
Click Make a copy in Google Docs, then confirm on the Google page. The copy is saved to your own Google Drive.
State each concern as a fact with a date or number, then rate its likelihood and impact and name one owner for each response.
Rewrite each expectation as a measure with a baseline and a target, and spread the first actions so at least one is due in the first two weeks.
Add the people who will support the employee, schedule the review dates, and record who will check each one.
Delete the guide page, then share the plan with the employee and HR or present it at the kickoff meeting.
Most plans run 30 to 90 days, with a review at the midpoint. The sample uses 12 weeks with formal reviews on day 30, day 60 and at the end.
Yes. Have HR check the wording, the evidence for each concern and the review dates. Employment rules differ by state, so confirm local requirements first.
No. Hartwell Partners, the people, the email addresses, the phone numbers and every figure in the sample are fictional. Replace them with your own details before sharing.
