A careless reference can harm the person it describes and expose the person who writes it. This template keeps to facts: employment dates, the roles held, training completed, attendance and conduct, set out on a single timeline that a recipient can read in a minute. A second page shows who verifies and signs.
The sample is a reference for Amira Hussain, who worked for Ironvale Care, a fictional care provider, from 7 June 2021 to 30 May 2025. It shows three roles, a 98% attendance record, no warnings and a team of six. Replace the dates and roles with HR record data, choose two real examples, and delete any line your policy does not allow.
Click Make a copy in Google Docs, then confirm on the Google page. The copy is saved to your own Google Drive.
Replace the employee, dates and roles with data from the HR record, and check them with HR before you go on.
Choose two real examples that show strengths relevant to the job being applied for, and delete any line your policy does not allow.
Update the contact cards and the responsibility table to match who in your organisation may give references.
Delete the guide page, then sign the letter and send it from your work email.
No. Ironvale Care, Amira Hussain, the other people and every figure are fictional sample content. Replace them with facts from your own HR records.
Only where your policy allows and the employee has agreed. Leave out health, family circumstances and protected characteristics, and check with HR before you send.
Use one or two concrete results that match the role being applied for, such as a 30% fall in missed visits or leading a team of six. Avoid general praise.
