This free Google Docs template gives staff a purchase requisition form that sets out exactly what is needed, what it costs and which budget pays for it. It covers the requester, the items with quantities and unit prices, the cost code, the justification and the approval chain. A completed requisition is the first step of the three-way match that lets accounts payable pay without argument.
The sample is a requisition from Cinderfield Capital, a fictional firm, for laptops, monitors and desks for a new team in Leeds. Its names, prices and reference numbers are invented. Replace the items, quotes and budget code with your own, and confirm the approval chain with finance before the form is submitted.
Click Make a copy in Google Docs, then confirm on the Google page. The copy is saved to your own Google Drive.
Complete every line with a quantity, a unit, a unit price and a supplier.
Attach one supplier quote for each line above £500, and reference its number.
Confirm the cost code with your budget holder before you submit the form.
Delete the guide page, then send the form to the approvers in the order shown.
No. A requisition is an internal request for approval. The purchase order is the binding instruction to the supplier, and it is raised only after the requisition has been approved.
No. Cinderfield Capital, the people, prices, budget codes and reference numbers are fictional sample data. Replace them with your own details before you submit the form.
Set your own thresholds. This sample requires a second approver for orders above £10,000, and the finance team should confirm the limits for each budget holder.
