Audit findings only matter if the actions that follow them are finished and can be shown to be finished. This template reports on earlier findings in seven pages: the prior actions and their status, the evidence behind each closure, the overdue items, the escalations and the financial effect of recovery. It separates what was tested from what management says, so the committee can see both.
The sample is for the fictional Ravenmoor Group, where 13 of 17 prior actions are closed, 3 of 22 controls failed testing and 9% of purchase orders were raised after the invoice date. Replace the findings, ratings, dates and names with your own evidence, and do not mark anything closed without a filed record.
Click Make a copy in Google Docs, then confirm on the Google page. The copy is saved to your own Google Drive.
Replace the Ravenmoor Group name, the audit title and the period on the first page with your own.
Write the rating scale, then rate each finding and link it to a named owner, a due date and filed evidence.
Delete the guide page, check that every closure has evidence on file, then share the report with the audit committee.
Yes. Click Make a copy in Google Docs and the file is saved to your own Google Drive, with no sign-up and no charge.
No. Close it only when the audit team has seen filed evidence that the control now works, and record the date of that check.
No. Ravenmoor Group, its auditors, managers, figures and recoveries are fictional sample data. Replace them with your own before use.
